Act 24 - Arrest/Conviction Report and Certification Form
 
Act 24 of 2011 contains a number of significant changes to the Pennsylvania Public School Code that are designed to enhance the safety of school children. Among the changes, Act 24 amends Section 111 of the School Code to expand the list of prohibitive criminal offenses and ensure that then-current school employees who may not have been subject to a previous background check were required to provide assurances that they had not been previously arrested or convicted of a Section 111(e) offense.
 
Additionally, Act 24 requires that all school employees complete and return to their school administrator a PDE Form 6004 (PDF) within 72 hours of any arrest or conviction of an offense listed in Section 111(e). If an employee refuses to submit the form, Act 24 requires that the School Administrator submit the employee to a new background check under Section 111.

  • An additional form will need to be completed for any PAST employer that is a school entity.
  • An additional form will need to be completed for any PAST employer in which the paid position involved direct contact with children.

School Personnel Health Record
 
The Pennsylvania Public School Code Section 1418. Medical Examinations of Teachers and Other Persons requires that "teachers, janitors, cooks and other cafeteria help and all others employed at schools have a pre-employment medical examination. The law does not allow modification of this section.
 
All new employees are to have the Health Form and a Tb test completed prior to their first day of work.

The Derry Area School District provides faculty, students and staff the opportunity to connect to the world wide computer network called the Internet. The Internet is to be used for educational purposes as an educational research tool. The Derry Area School District supports confidentiality of information through the Family Educational Rights and Privacy Act (FERPA) and Internet Content Filtering guidelines through the Child Internet Protection Act (CIPA). The Internet will be used to support the functions of the Derry Area School District, its curriculum, the educational community, and projects between schools, communication and research for school district administrators, teachers and students.
